This vehicle crack my glass and several other vehicle and proceeding to drive away after several car tried to get her to pull over she seem drunk or under the influence and I follow as far as I could until she pulled over and drove backward up a ramp... I called the company and he accuse me of lying about her having rocks or gravel falling from the top of the trucks never got his name but his phone number 609)536-0438 I would appreciate if somebody would be able to call me and address this issue this is not fair that their not closing or covering there equipment correctly.. they could have cost me my life.. Safeway van lines dsp for Amazon in Little Rock Arkansas is a joke. This isn’t the first time I’ve witness unsafe driving due to being under the influence of drugs or alcohol. The employees or drivers are open about smoking marijuana in parking lot pre shift and during shift. Very unprofessional and very unsafe.

What makes a helpful report?
Record the details when it is safe to do so. You do not need a perfect account, but clear observations are more useful than assumptions.
Identify the right vehicle
Use the company name, MC or USDOT number, or the license plate and issuing state. Double-check that you selected the correct company or vehicle before submitting.
Add time and place
Include the incident date, road or intersection, nearby exit or mile marker, and direction of travel. A precise location gives readers meaningful context.
Describe the sequence
Explain what you saw in the order it happened. Describe the behavior directly and distinguish what you observed from what you could not confirm.
Include relevant images
Add a photo if you already have one that supports your account. Avoid unrelated faces, private documents and personal contact details. Do not take photos while driving.

A few answers to help you prepare a clear and useful report.
How do I report an unsafe truck driver?
Select “Add a report,” find the company or license plate, and enter the incident date, location and description. Choose the relevant incident types and attach any appropriate photos before saving.
What if I only know the license plate?
The report form supports a license plate search. Select the issuing state and enter the plate number, then check the matching vehicle details before continuing.
Does Dcontrol forward reports to authorities?
A report on Dcontrol is a community contribution. To submit an official federal safety complaint, use the FMCSA complaint portal directly. In an emergency, call 911.
Are community reports official findings?
No. Reports describe authors’ experiences and may include disputed or incomplete information. Read the full account and available responses, and verify important details independently.
What should I leave out of a public report?
Keep your account focused on the incident. Avoid threats, speculation and unrelated personal information such as home addresses, private phone numbers or identity documents.
Can I add a photo?
Yes, the report form supports image uploads. Use relevant images you are entitled to share and check that they do not expose unrelated personal information.

On last Friday I followed behind the truck getting on 675 from forest parkway and the truck had a tire blow out on the right side the outside tire and some of the material blew back and hit my windshield on the right side and cracked it
